The following is a list of the documents you may need to complete your application:

  • General Information Card
  • Election Form (federal tax withholding)
  • Important Notice (return to work)
  • Partial Lump Sum – Form B
  • Partial Lump Sum – Form E
  • Spouse Card
  • Marriage Certificate
  • Spouse’s Birth Certificate
  • Disability Application Card
  • Social Security Award Letter
  • 401(k) Distribution Form

When you decide to retire or merely contemplate retirement, it is strongly recommended that you contact the Pension Fund Office and discuss your retirement options.

Important Note: This is an outline of the principal plan provisions of the Plumbers’ Pension Fund Local 130 U.A. Plan and it is not intended to completely describe the Plan provisions. In the event of any discrepancy between this outline and the Plan, the Plan Document shall govern. For further information, please review your Summary Plan Description or contact the Fund Office.
